Service Coordination

Accessing Service Coordination

Developmental Disability Advocates employ some of the most skilled and caring service coordinators in the region. They will meet with any individual with a developmental disability, age 3+, that lives in Jefferson County.

Eligible disabilities include:  intellectual disability, cerebral palsy, head injury, autism, epilepsy and certain learning disabilities.

Upon meeting, a service coordinator will conduct an assessment of what services could benefit the individual with disabilities and develop a plan with the individual and their primary caretaker.

Service Coordination’s Goals

Developmental Disability Advocates partner with the Department of Mental Health to meet the needs of individuals in a more timely and efficient manner. It also helps to reduce caseload sizes and develop funds through targeted case-management.

As a result, we have been able to enhance service coordination in Jefferson County and provide quality services to individuals and families.

We are Committed to Listen & Inform

  • We will ensure you have an assigned service coordinator within our agency that aligns with your perspective.
  • We will keep you updated on information that could have an impact on the services you receive and return messages in a timely manner.
  • You have the ability to take the lead in planning the support you need, as well as a role in planning a budget. Our door is always open for questions, concerns or compliments.

Funding

Services for individuals receiving Service Coordination from DD Advocates are funded through a combination of federal, state and local funding.  Medicaid is the largest funder of services and can take 2 forms – purchase of service or waiver.  Missouri operates 4 Medicaid waivers for people with developmental disabilities.  Services are also funded through the Missouri Department of Mental Health – Division of Developmental Disabilities.  This takes the form of general revenue funds, Medicaid match dollars and/or state funding specifically for individuals with autism and their families.  Finally, services may also be funded at the local level from Developmental Disability Advocates through county property tax dollars distributed to agencies who support people living in Jefferson County.  Individuals may also be eligible for funding or services from a variety of federal/state benefits and/or local resources and are assisted by their Service Coordinator in accessing them.
